Our In-Home Family Support (IHFS) Program is geared towards supporting participants and their families in their homes as well as in the communities in which the live. Community integration forms an integral part of the life of each participant we support. Through our Participant-centered programs we strike a balance between the individual risks and abilities Community Vision Development Programs, LLC focuses on participants’ potentials to help them achieve independence and self-esteem for a better life at home with families and socialization in the community. A Participant’s Individualized Service Plan (ISP) sets forth specific achievable milestones called goals which our Direct Service Professionals work closely with the participant to achieve. Goals vary from participant to participant depending on the person’s specific needs. Among the goals common to most participants are but not limited to the following:
- Community integration activities
- Building Relationships
- Independent daily living skills
- Money Management skills
- Problem solving skills
- Relaxation techniques
- Making Healthy Choices
- Discovering talents and individual abilities
