Community Vision Development Programs, LLC

INHOME FAMILY SUPPORT SERVICES: Staff is sent to the individual’s home to provide training and support to a consumer and his/her family, including extended family, in the family home and in the community. In-home programs are designed to increase the family’s ability to care for and support the person in the family home as well as in the community.

SUPPORTED LIVING SERVICES:
Waivered Services:
Individuals with disabilities live in a small group up to four in a home away from home in  a community of their choice.  Individuals are given opportunities to discover their full potentials through their strengths, gifts, talents and abilities and are encourage to engage in self-advocacy.           

Elderly Waivered Services:This is an alternative to nursing home.  One to four seniors live in a home setting in a community of their choice while receiving execellent services from professionals and dedicated staff with a passion of making a life-long difference in the lives of those they serve.

PERSONAL CARE ASSISTANT SERVICES: Qualified PCA work with individuals providing human assistance and support to persons with disabilities and seniors living with family or independently in the community. Staff are trained to care for the elderly and others with speical health care needs.

CASE MANAGEMENT: Assist individuals in locating and gaining access to necessary waiver services regardless of funding source; we coordinate the plan of care, monitor services so that individual consumer’s health and safety are assured, and conduct periodic reassessments.

COMPANION SERVICES )
Staff trained in home management provide services base on the individual services plan, provide residential services that include general household activities such as meal preparation, routine household care, shopping and errands, arranging transportation, emotional support, and monitoring safety and well being of all consumers.

HOME-HEALTH AIDE: Well trained HOME HEALTH AIDES, NA/RS and PARANURSING PROFESSIONALS take care of consumers needs in areas such as:
Housingkeeping, Meal Preparation, Eating, Toiletting, Bathing, Grooming, Dressing, Laundry, Grocery Shopping, Errands, Rehabilitation Exercises, Personal Hygiene, etc.

HOME HEALTH CARE (Both Long Term/Short Term): This is an ALTERNATIVE TO NURSING HOMES where consumers receive services from qualified PARANURSING PROFESSIONALS and other trained staff as they live in a therapeutic home setting in the community of their choice.

EXTENDED PERSONAL CARE (with skilled/professional nursing care services
catheter care, wound care, diabetic management, end of life care, medication management, tracheostomy care, analgesics, hyperalimentation administration, antibiotics administration, health assessment, teaching, treatments
